Exceptional Student Services accepts feedback from the public concerning the Individuals with Disabilities Education Act (IDEA) Part B State Application, the State Performance Plan and Annual Performance Report (SPP/APR), special education data indicators, proposed legislation, and proposed or revised policies and procedures. This form allows a user to submit and categorize input. If there are multiple comments regarding different categories, please submit a separate comment for each category.

This form has certain fields required and those are notated with an asterisk. Other fields are optional but are helpful for our unit determining the representativeness of public comments to ensure we are capturing diversity within the state.
